This is an alternate twist on the Doctor and the fob watch we were introduced to in "Human Nature", "Family of Blood" and later "Utopia". John Smith retains no memory of his identity as the Doctor, individuals he's come to know over the centuries nor how he came to be in present day London beyond what was pre-established by the fob watch. Much like in "Human Nature" he continues to receive bits and pieces of his life as the Doctor via dreams, but has begun taking a self-prescribed regiment of antipsychotics to stave off the 'episodes'.

He is currently working as a Doctor of emergency medicine at St. Bartholomew's Hospital in Smithfield. A restive work-o-holic by nature, he rarely sees the outside of the hospital, preferring the breakneck pace of his career over the maddening mediocrity of anything close to resembling a personal life.

The fob watch, at present, is lost.
